Transition Plan Agreements (TPA) allow for a foster care provider to take part in a transition plan for a child from a facility into the provider’s home. Transition Plan Agreements can be utilized with State, Tribal, or Nexus Path providers. Providers must be in full compliance with license requirements and cannot have an open child protection services (CPS) assessment. If at any time, a CPS assessment opens during the TPA timeframe, the agreement will end the date of the opened CPS assessment. Upon closure of the CPS assessment, the Transition Plan Agreement can be put back in place with a new SFN 1197, if the team agrees the family can meet the needs of the child and it is still in the child’s best interest.
Discharge planning must begin on the first day a child is placed in a treatment facility (QRTP or PRTF). Once a licensed provider is identified as the discharge placement, they may be asked to participate in a Transition Plan Agreement. If the selected family is licensed, reimbursement is available to help support the transition period.
